    At Here’s To Life, we believe that lasting change begins when people are supported as whole individuals, not just as patients or program participants. Our Whole Person Approach is a holistic framework that guides every aspect of our prevention and support services. It recognizes that individuals are complex, with interconnected needs across physical health, mental well-being, emotional resilience, social connection, and purpose.

    Through our Circle of Care Model, we deliver integrated, wraparound services that reflect the full reality of our clients’ lives. This model is especially impactful in serving African American MSM communities, where systemic barriers often require more than a single intervention to create long-term impact.

    Our Circle of Care includes:

    Prevention Services

    Culturally responsive education, HIV/STI testing, and risk reduction strategies to empower individuals before crises occur. Brothaz Helping Brothaz, Seasoned Brothaz Network, and R.I.C.E.

    Access to Medical Care

    Navigation and linkage to affirming, high-quality HIV and primary care services that support viral load suppression and overall health.

    Recovery Support Services

    Evidence-based substance use recovery programming that promotes healing, accountability, and community.

    Mental Health Services

    Trauma-informed counseling and group therapy to address emotional and psychological well-being.

    Housing Services

    Assistance with stable, supportive housing to reduce homelessness and housing insecurity—critical social determinants of health.

    Workforce Development

    Job readiness training, career coaching, and employment support to foster independence and purpose.

    Linkages to Social Determinants of Health

    Coordinated referrals and partnerships that address food insecurity, transportation, legal aid, and other essential needs.
