Lead Substance Abuse Counselor

Position Summary
The HOPWA Lead Substance Abuse Counselor providesclinical leadership and direct substance use treatment services to individuals and families living with HIV/AIDS who are enrolled in HOPWA housing programs. This position addresses substance use disorders as a critical factor impacting
housing stability, health outcomes, and overall wellness.The Lead Counselor is responsible for delivering evidence-informed treatment, supervising counseling staff, ensuring regulatory
and clinical compliance, and integrating substance use services into broader
housing stabilization and healthcare efforts at Here’s To Life, Inc.Essential Duties and Responsibilities
Clinical Service DeliveryProvide individual and group substance use counseling utilizing trauma-informed, harm reduction, and recovery-oriented approaches.
Conduct comprehensive substance use assessments, screenings, and diagnoses consistent with licensure scope.
Develop, implement, and monitor individualized treatment plans that support recovery, housing retention, and stability.
Provide relapse prevention planning, crisis intervention, and recovery support services.
Clinical Leadership & Supervision- Provide clinical supervision, coaching, and support to substance abuse counselors and peer recovery staff.
- Review clinical documentation for quality, compliance, and adherence to treatment standards.
- Support staff development through training, case consultations, and performance feedback.
Integration with Housing & Supportive Services
- Collaborate closely with housing managers, case managers,and mental health staff to address substance use barriers impacting tenancy stability.
- Participate in multidisciplinary case conferences and coordinated care planning.
- Support residents during housing or recovery-related crises that threaten stability.
Compliance, Documentation & Quality Assurance
- Ensure all clinical services comply with HOPWA supportive services guidelines, state licensing requirements, and agency policies.
- Maintain accurate, timely, and confidential clinical records and service notes.
- Monitor data entry into HMIS and behavioral health records systems.
- Assist with audits, monitoring visits, corrective action plans, and grant reporting.
Community Partnerships & Referrals
- Build and maintain relationships with detox facilities, residential treatment centers, outpatient providers, and recovery resources.
- Facilitate warm handoffs and referrals for clients requiring higher levels of care.
- Represent the agency in community coalitions and substance use prevention initiatives when assigned.
Required Qualifications & Qualities
Education & Licensure- Master’s degree in Social Work, Counseling, Psychology,or a related behavioral health field required.
- Current Georgia licensure or certification required(e.g., LCSW, LPC, LMFT, CADC, or equivalent; supervision credentials preferred or required based on role).
Experience
- Minimum of 4–5 years of experience providing substanceuse counseling or treatment services.
- At least 2 years of clinical supervision or teamleadership experience preferred.
- Experience working with individuals living with HIV/AIDS,co-occurring disorders, trauma, or housing instability strongly preferred.
Knowledge & Skills
- Strong knowledge of substance use treatment modalities,harm reduction, relapse prevention, and co-occurring disorder treatment.
- Familiarity with HOPWA supportive services and HUD-fundedprograms preferred.
- Excellent clinical judgment, documentation, andorganizational skills.
- Proficiency with electronic health records, HMIS, andMicrosoft Office.

Position Summary
Here’s to Life, Inc. is excited to announce that we are hiring Prevention Navigator(s) for Full-Time and Part-Time positions. Here’s to Life (HTL) is one of Atlanta's leading providers of specialized substance abuse treatment specific to the HIV-positive Men who have Sex with Men (MSM) and Transgendered communities.
The Prevention Navigator is a full-time community-based HIV and Substance Abuse case management position. This position works alongside a dedicated specialized recovery team who work with Men Who Have Sex with Men (MSM) and Transgenders that are experiencing substance abuse challenges and/or high risk for HIV or living with HIV/AIDs. The Prevention Navigator works closely with the target population, assisting with resource linkages and system navigation while promoting recovery.
Our approach encompasses a multidisciplinary team of licensed addiction professionals, social scientists, and peer-led recovery coaches.
Here's To Life is a holistic harm reduction model with a whole-person approach that provides Services-On-Demand, with a focus on behavioral health outreach, to facilitate relationships between healthcare providers’ practices and agencies addressing the various social determinants of health.
Education and Training: A lived experience or bachelor’s degree in social work, or other social services-related field.
Experience: At least one (1) year of human service experience, working with either an adolescent and/or transitional age population. Service coordination or case management experience strongly preferred but not required, or an equivalent combination of education and experience as approved by the Executive Director.
Duties and Responsibilities
- Assists the Lead Prevention Navigator with data collection and entry.
- Attending family and treatment team meetings.
- Coordinate meetings with community partners I.e. CBO’s, Bar, Clubs Promoters
- Provide advocacy and support during meetings and collaboration with additional external providers.
- Assist individuals with applying for health benefits, employment, schooling, housing, etc.
- Demonstrate and support the HTL mission and values through performance of job duties and responsibilities.
- Attend training and supervision as required by applicable regulation, HTL, and program-level policies and procedures.
- Adhere to, implement, and promote program-wide philosophy and approaches.
- Participate actively in one’s own professional growth and development plan.
- Maintain consistent attendance as a demonstration of commitment to the program's safety and welfare.
- Performs other related duties as assigned.
Job Type:
Full-time $41,000 Annual Salary
Part-Time #25 hours Pay: $15.00 - $17.50 per hour
