What is NURTURE?
The NURTURE Project is a holistic Linkage to Care program designed to support individuals by connecting them to essential services that promote long-term health and stability. Through a person-centered approach, the project addresses key areas such as sexual health, behavioral health, housing support, workforce development, and more.
As part of the intake process, participants complete a comprehensive needs assessment survey to help identify their unique goals and challenges. This ensures that each individual receives personalized referrals and support services tailored to their circumstances.
Whether you're seeking help with PrEP access, mental health counseling, job readiness, or housing navigation, the NURTURE Project is here to walk alongside you on your path to wellness.

Here’s to Life Prevention Services empowers African American MSM with culturally responsive, evidence-based education and support. Our interactive classes on substance use, HIV, and hepatitis C combine science with compassionate delivery to build awareness and trust.Partnering with the NURTURE initiative, we use the NURTURE Survey to tailor our programs to community needs. Beyond education, we offer comprehensive HIV/AIDS support, including medical care linkage, mental health and substance-use treatment, housing and employment assistance, prescription subsidies, and PrEP/PEP navigation.
We train local providers annually for culturally competent care and increase our reach with targeted social-media campaigns, aiming to boost HIV, STI, and hepatitis testing by 10% each year. At Here’s to Life, prevention means transformation, supporting individuals on their journey to healthier, more stable lives.

At Here’s to Life, we understand that navigating health systems and prevention services can feel overwhelming—especially when dealing with stigma, barriers to care, or limited resources. That’s why our Prevention Navigators offer personalized, one-on-one support to help you access the tools and services you need to protect your health and live well.
Whether you're seeking HIV testing, access to PrEP or PEP, Hepatitis C support, or resources for safer substance use, our trained navigators do more than hand out referrals—they walk with you, advocate for you, and stay connected throughout your journey.
